Scrutinized the Ch.off and Master port watchkeeping order etc. Opening meeting 0730 with all deck lights ON including emergency lights.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">1. Documentation etc etc\u2026last PSC report\/USCG\/ SIRE report.<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">2. CMS master to show Ch. Off and CE to show 2\/E report progress or completion. verified all good.<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">3. CE: \u201chow do you report when some equipment fails related to Class?\u201d<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">4. Last Mooring break test report \/ how it was done\/ where is equipment located.<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">5. Bunker check list each and every page check<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">6. LO reports<\/h4>\n\n\n\n<h5 class=\"wp-block-heading\">7. Emg gen test first SPRING START then battery start.<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">11. Oiler are included or not?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Where is mooring plan file etc..<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When Bsn came to CCR interview started.:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">a) Bsn where is your mooring station?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">b) How many people with you?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">c) Who operates winch?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">d) Bsn what is your duty in enclosed space?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">e) What do you check ? Portable meter<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">f) Who else checks?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">g) Who all sigh Enclosed space permit?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">h) What equipment\u2019s to bring?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">i) Before sign permit what is done? Meeting \/plan etc..<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">j) How do you check gas? Stop fan wait 5 to 10 mins etc..<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">k) How do you report if you are entering in COT?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">l) Asked Ch. Officer to provide Mooring plan showed to Bsn and asked him \u201c you have signed training record did you read the plan?\u201d Bsn was blank..no answer<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">m) Inspector asked\u201d after CARGO LOADNG if shore team ask for sample\/ ullage of COT Ch. Off gave responsibility to Bsn to handle shore team accordingly what is your action?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Bsn response: wait 30 mins because FIRE HAZ.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inspector asked \u201cwhy\u201d Bsn reply to relax time cargo after loading.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inspector asked exact reason to 3rd officer. and he replied properly.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Ch. Off interview:<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">1. ESP RA\/ permit how it is filled up ?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">2. How TC was carried out.<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">7. Which tank was used to retain wash water?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">8. Veriifed ORB for Decanting.<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Watchkeeping during port stay discussed.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">3rd officer:<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">How many gas instruments on board.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Show one instrument and calibrate.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">LUNCH BREAK<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">ER<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">2\/E : Show me HHC hyd hose ROB.<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Compare with PMS<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inspector asked for location and wanted justification why 1 Complete set of hose is mentioned in PMS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inspector gave remark (process) each hose should me mentioned with part no. and kept secured for accidental use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Show PMS eng. Alarm tried out.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Engine fitter interview:<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Hotwork procedure in designated area.?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">How much distance of Oil tank should be for HW?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Who completed HW CH LK ?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What is procedure if need to carry HW outside.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What all is required to propose HW outside.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bring Welding PPE<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Checked Helmet\/glass\/gloves\/ apron ( it was all in use so inspector was satisfied)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cFitter what all re check you conduct before carrying out HW\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">3E:<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">Show UMS CH LK\/ arrival departure \/ CE standing order\/ CE port order book.<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">ETO: ER insulation mat certificate \/ 220 V\/440V low insulation.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Alarms:<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">1. Random checks of Eye wash in chem store\/boiler\/stp<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">8. Random checks of EEBD<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">9. ME emg. Man plat Lamp test.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">10. Hypermist panel Lamp test.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">ACCOMODATION AREA<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">1. Foam room \/ level gauge\/ had air bubble in glass verified if GG is without air bubble.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">BRIDGE 2ND OFFICER\u2026.<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Completed with 1 remark about deck watch and 1 remark about HHC spare set hose not 1 set not kept securely (in box with pad lock) for preventing accidental use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Positive remark.
