Rajan had entered one of the vessel’s oil tanks as part of a routine cleaning operation when he allegedly began feeling dizzy before collapsing inside the enclosed space.

Crew members immediately initiated an emergency rescue operation and successfully recovered him from the tank. The vessel’s emergency response team provided first aid and onboard medical assistance, but despite their efforts, he failed to regain consciousness.

The ship’s management then contacted company doctors via satellite communication for remote medical guidance. After evaluating the situation, medical personnel confirmed that Rajan was unresponsive and showed no signs of life.

As the tanker was operating close to Mumbai Port limits, the crew informed local authorities. The deceased was transported ashore at Bhaucha Dhakka on June 3 and subsequently taken to JJ Hospital, where doctors officially declared him dead.

Police investigations revealed no signs of foul play or external injuries. Statements recorded from family members and witnesses indicated no suspicion regarding the circumstances of the death. An inquest conducted in the presence of independent witnesses also found no suspicious elements.

Authorities have registered an Accidental Death Report (ADR) under Section 194 of the Bharatiya Nagarik Suraksha Sanhita (BNSS) and have launched further inquiries into the incident. A post-mortem examination was conducted before the body was released to the family for final rites.

The tragedy serves as a stark reminder of the persistent risks associated with enclosed space entry onboard ships. Tank cleaning operations remain among the most hazardous tasks in the maritime industry, requiring strict adherence to safety procedures, atmospheric testing, proper ventilation, rescue preparedness, and continuous monitoring to prevent loss of life.

As investigations continue, the maritime community mourns the loss of an experienced seafarer whose death underscores the critical importance of enclosed space safety at sea.

A tragic incident aboard an oil tanker operating off the Mumbai coast has once again highlighted the dangers associated with enclosed space operations at sea.
