Here&#8217;s what the data \u2014 and a tragic incident \u2014 tell us.<\/em><\/p>\n\n\n\n<h4 class=\"wp-block-heading\">What Is LOTO \u2014 and Why Does It Matter?<\/h4>\n\n\n\n<p>Lock Out Tag Out (LOTO) is a safety procedure designed to prevent the accidental start-up or energisation of machinery during maintenance or inspection work. Unaware of the ongoing maintenance, another crew member activated the ballast system. The motor started unexpectedly.
