Capt. Md. Sha… boarded vessel and after initial gangway briefing proceeded to Master’s office.
Opening Meeting
Opening meeting conducted with ship management team. Inspector explained scope of inspection and advised he will check equipment and procedures in deck and engine in detail. He emphasized stop-work authority if any unsafe act observed. Also requested to inform any port restrictions before testing equipment such as emergency generator, lifeboat/rescue boat or fire pumps.
Documentation Review
With Chief Engineer:
• Detailed check of all engine documents
• Last 1 year defect list reviewed with cross questioning
• Bunker reports
• Lube oil sample records
• Engine room manning records
With Master:
• Rest hour & familiarization records
• Supernumerary (superintendent/service tech) visit records last quarter with safety familiarization
• Electrical equipment familiarization records
Bridge Inspection
• UKC policy (Under Keel Clearance), echo sounder & ECDIS records checked
• Abort points & contingency anchorage verified
• Passage plan deviations checked on ECDIS and paper charts
• GMDSS test records
• Navigation warnings verified
Deck Round & Equipment
• MOB marker and bridge equipment
• Provision crane condition, limit switches & leak check
• Vents and flaps rubber packing condition
• Emergency generator started and put on load
• Lifeboat physical condition & lights tested
• Rescue boat engine run & davit limit switches checked
• Liferaft launching explained by gas engineer
• Garbage area inspected
• Mooring stations: markings, brake marks, rope condition, correct turns on bitts
• Accommodation outer condition
• Tank dome & manifold valves/gauges condition
• Compressor room leak check, gauges, panel lamp test, safeguards & ventilation, entry procedure
• PV valves & MARVS maintenance records verified
• Mooring winch lever neutral return checked
• Bosun store & bilge alarms condition
• Paint store sprinkler procedure explained
• Emergency fire pump tested with pressure verified
• MSDS & eyewash expiry checked
• Gangway markings verified
Crew Interaction – Deck
• Daily plan meeting contents
• SCBA checks & demonstration of wearing
• ESD locations and actions
• Nearest fire extinguisher awareness
• Routine deck round checks
CCR / Cargo Control Room
• Detailed cargo plan verification
• Last pre-arrival alarm log

• BWTS principle & test
• Enclosed space entry records & last permits checked in detail
• Cargo plan signatures including new joiners
• Gas meter calibration & log
• Mooring records & maintenance intervals
Accommodation / Galley
• Galley hygiene and cleaning
• Chief cook explained firefighting medium for galley on gas tanker
• Smoking room self-closing ashtray arrangement verified
Engine Room / ECR
Inspector carried out detailed interaction and checks:
• Insulation monitoring settings verified (220V & 440V)
• OWS alarm & bilge alarm checks
• Lifting gear condition and records
• Engine room watch schedule during manoeuvring
• Duty engineer actions in case of loss of propulsion
• Emergency plan location shown in Company SMS
• Arrival and departure watch level entries in logbook verified and signed by Chief Engineer
Questions asked by inspector:
• Why insulation limits set at 5 MΩ for 440V and 2 MΩ for 220V
• How to check lifting gear condition and what inspections carried out
• Actions in case of loss of propulsion and how to refer checklist in company SMS
• Engine logbook entries and night orders
• Seal records and engine room rounds
• OWS test records
• Steering gear checks
Overall Impression
Inspection was very detailed and practical-oriented. Inspector focused on crew knowledge, company SMS familiarity, documentation accuracy and equipment condition. Professional and cooperative approach throughout with strong emphasis on safety awareness and proper procedures.
