1. Fire Safety
- Observations: Fire dampers not closing fully, defective emergency fire pumps, and frozen isolation valves.
- The “Jan 2026” Trend: High focus on fixed CO2 system maintenance records and the condition of fire-fighter outfits (SCBA bottle pressure and mask integrity).
2. Safety of Navigation
- Observations: Outdated ENC charts, faulty VDR (Voyage Data Recorder) sensors, and improper passage planning.
